Waivers, questionnaires and welcome packets, signed and kept

Questionnaires, waivers and welcome packets. Build a form from questions, documents to read and a signature, give it to everyone, a group or chosen members, see who has not finished, and keep each signed copy. An intake link lets people fill in a short form before they join.

What Forms does

Every organisation has paperwork that must come back signed: the pool waiver, the photo consent, the handbook acknowledgement, the field trip form. It goes out as an attachment and comes back as a photo of a page, or not at all. This app keeps the form, the answers and the signature in one place and shows who still owes one.

A form is a list of questions and document steps. A document step shows a PDF or other file the organizer uploaded, some text, or a link, and asks the member to confirm they have read it. If the form asks for a signature, the member types their name and agrees to a statement, and the exact wording and document they saw are fixed on their copy. Questions cannot be changed once a form is open.

Submissions are private. A member reads their own; a reviewers group, chosen by a hub admin, reads everyone's and sees who has not started. Nobody else can, not even an admin outside that group. A file a member attaches, such as a photo ID or a certificate, opens only for them and that group too. In a classroom or team space only the teacher reads them, once added to that group, and families never see one another's forms.

How it works

  • Short and long answers, choices, yes or no, numbers, dates, a file the member attaches, and read-and-acknowledge steps with an attached document.
  • A typed signature with the time and the exact wording it was given against.
  • For everyone, one group, or chosen members. A due date, a notification when it opens, and a reminder button for whoever is behind.
  • Save a draft and finish later; a submitted form cannot be changed.
  • A reviewers group reads submissions and prints or saves any one as a PDF.
  • An intake link: a name, email, phone and note from someone who is not a member yet.
  • Open forms with a due date appear on Today.

A signature here is a typed name, a time and a record of the wording agreed, kept inside the hub. It is not a certificate-based electronic signature, and the intake link's fields are fixed: a name, an email, a phone number and a note.
